Local Adult Library Registration

If you are 16 or older and live; own property/pay taxes; teach at or attend public school in Berlin, Calais, East Montpelier, Middlesex, Montpelier or Worcester you may continue with this form.

Please note online registrations are temporary and cannot be renewed until you bring proof of local residency to the Library and pick up your library card at the main desk.

If you are Kindergarten age through 15, please fill out a youth registration.

You may purchase a Library card if none of the above categories apply to you. To purchase a Library card, please complete the form here.
